Abstract : Background: Type 2 diabetes mellitus (T2DM) is a global disease with a compelling impact on both developed and developing economies. The global burden of T2DM among adults has grown following the abandonment of healthier lifestyles, physical inactivity, age, and family histories. Positive patient outcomes attributable to effective treatment and management are largely shaped by illness perceptions and medication adherence. Aim: To assess the level of illness perception and medication adherence and its relationship among adult patients with T2DM in Jeddah, Saudi Arabia. Methods: A descriptive, correlational, cross-sectional design study was conducted in a convenience sample of 365 patients with T2DM at the Jeddah Care Centre for Diabetes and Hypertension at King Abdul-Aziz Hospital in Jeddah, Saudi Arabia. Data collection tools were the sociodemographic and clinical questionnaire, the Brief Illness Perception Questionnaire (Brief IPQ), and Morisky Medication Adherence Scale (MMAS-8). Descriptive statistics and Pearson correlation were used for data analysis. Results: The mean age of participants was 50.9 ± 15.87 years (range 18-81), and 53.4% were female. Respondents showed a high perception of diabetes timeline as a chronic disease that will stay forever (mean 7.45 ± 3.17). Understanding of their illness was low (mean 2.73 ± 2.38); likewise, they had a negative perception regarding the effectiveness of the treatment (mean 2.27 ± 2.52). There was a significant inverse correlation between the Brief IPQ score and the MMAS-8 score (r = -0.489; p<0.001), and 62.2% of participants had a low medication adherence rate. Conclusion: The level of illness perception influences medication adherence behaviour among adult patients with T2DM. Nevertheless, the effect determines the condition’s recovery process, management, or severity. Recommendations: Illness perception assessment should be included in the treatment plan for T2DM to improve adherence to effective self-care management. Further research is required to examine the effectiveness of illness perception-based interventions to improve treatment adherence in the Saudi context.
