Abstract
:
The purpose of this study was to assess the effect of isokinetic training program of knee extensors on changing the excitability of motoneuron pool, with consequent inhibition of spasticity and improvement of knee extension during gait of spastic diplegic children. The study included thirty patients from both sexes, with age ranged from six to eight years. They were selected on basic criteria to confirm the grade of spasticity and absence of structural deformities. They were divided equally and randomly into two groups (study and control groups). The study group received a program of neuro-developmental technique (NDT), followed by suggested program of isokinetic training for the extensors of the knee joint, while the control group received the same (NDT) only. Treatment was conducted for five months, three days per week. H/M ratio of the soleus muscle, knee extension torque and angle of knee joint during sub-phases of stance phase of gait were measured pre and post the suggested period of treatment. Results collected at the end of treatment from the study group revealed a valuable reduction in mean values of H/M ratio, which was concomitant with significant increase of quadriceps torque and knee extension angle during stance phase of gait. These results were statistically highly significant in relation to the results collected from the control group. This can be attributed to the effect of isokinetic exercises on inhibition of spasticity with consequent improvement of muscular tension of the antagonistic muscles. The benefits afforded from this study indicate that application of isokinetic exercises in cases of diplegia can reduce the excitability of motoneuron pool in the spastic muscles, which was concomitant with improvement in muscular strength.
